Pathological mechanism and clinical manifestations of sacroiliac joint sprain

Sacroiliac joint sprain refers to the ear-shaped articular surface of the sacrum and ilium, which is damaged by external force and causes damage to the joint and its ligaments, resulting in local aseptic inflammation such as congestion, edema, adhesion, and causes local pain and functional impairment. So what is the pathological mechanism of this disease?

【Etiology and pathogenesis】

(1) Acute injury: A sudden slip, landing on one side of the buttocks, or a sudden twist when bending over to bear weight can damage the sacroiliac ligament. As the ligament is pulled, the ilium slides away from the corresponding sacral articular surface, causing the joint to twist and displace and synovial incarceration. It can also occur in women whose fetuses are too large, causing the pelvis to expand during delivery and cause sprains or even subluxation of the joints.

(ii) Chronic strain: Long-term bending over while working or lifting heavy objects can cause degeneration of the sacroiliac joint and lead to injury over time. During pregnancy, women's ligaments may relax and stretch, often causing sprains due to bending and rotational activities.

【Clinical manifestations】

(1) Pain in the lower back (sacroiliac joint), which is localized, persistent, dull pain that worsens with activity and exposure to cold, and may be referred pain to one side of the lower limb.

(ii) The movement of the waist is obviously limited, the patient's trunk is slightly bent toward the affected side, the affected lower limb is afraid to bear weight, and there may be limping.

(3) The affected side is afraid of bearing weight, resulting in staggering and slow movements. The abduction and external rotation of the affected hip are limited.
