The diagnosis of irritable bowel syndrome is mainly based on symptoms, but organic lesions must be ruled out. People over 40 years old, especially those with weight loss, black stools or bloody stools, bloody stools, and gradually worsening symptoms, should undergo a colonoscopy, mainly to rule out tumors.
