Under normal circumstances, it can occur in any part of the body where there is fat, most commonly on the back, neck, shoulders, abdomen, breasts and proximal limbs, and less commonly on the face, hands, feet and calves. Subcutaneous lipomas can be single or multiple, and the main manifestations are local protrusions of the tumor, soft texture, no redness or swelling, no tenderness, and a certain degree of mobility. examine 1. Histopathological examination The cross-section of lipoma is mostly light yellow in pathological sections. The tumor is composed of mature fat lobules wrapped by a thin layer of fibrous membrane. The capsule is thin and complete, and the fat cells are consistent in size and shape. There are fat lobules separated by trabeculae, and there are branched fibrous tissue and capillaries between the lobules. There are not many blood vessels in the tissue, and the capillaries are unevenly distributed. There may be a small number of scattered foam cells mixed in. Sometimes focal myxoid degeneration, calcification, ossification, hemorrhage, necrosis, liquefaction or xanthomatous changes can also be seen. There is usually not much fibrous tissue. If there is more, it is called fibrolipoma. When the pedicle of a larger lipoma is twisted, it may become cystic due to liquefaction changes caused by local blood flow obstruction. 2. Ultrasound examination B-ultrasound can accurately determine the location, size, texture, and whether it is solid or cystic. Color ultrasound can directly see the blood supply of the tumor. If the blood supply is abundant, it may be malignant. It is necessary to do color B-ultrasound or vascular puncture angiography. Color ultrasound or angiography can clearly determine its boundaries, whether the capsule is intact, and its relationship with surrounding large blood vessels, nerves and other important tissues. Causes The cause of subcutaneous lipoma is not fully understood. Common solitary lipoma is related to the recombination of chromosome 12. The incidence of lipoma is higher in obese people, those with diabetes and high serum cholesterol levels. Sometimes lipomas are present locally after trauma, but it is not clear whether trauma causes lipomas. Multiple patients may have a family history. |
