Treatment of cervical disc herniation

1. Non-surgical treatment

Non-surgical treatment is the basic treatment for this disease. It is not only suitable for mild cases, but also a guarantee for preoperative preparation and postoperative rehabilitation of surgical treatment. For cervical disc herniation, it mainly includes the following contents:

(1) Cervical traction: The patient can sit or lie down, and use a Glisson's belt for traction. For general cases, the weight should be smaller at the beginning, generally 1.5-2kg, and then gradually increased to 4-5kg. The traction time is 1-2h each time, 2 times/d, and 2 weeks as a course of treatment. For patients with severe symptoms, light weight supine continuous traction should be used, with a traction weight of 1.5-2kg, and 3-4 weeks as a course of treatment. If there are adverse or uncomfortable reactions during the traction process, traction should be suspended. Traction therapy is mainly suitable for lateral cervical disc herniation, and can also be used for central cervical disc herniation. However, if the pyramidal tract symptoms worsen during traction, surgery should be performed as soon as possible. In addition, during traction, avoid excessive flexion of the head and neck. This position may aggravate the compression of the posterior nucleus pulposus on the anterior central spinal artery, worsening the condition. During the entire traction process, the condition should be closely observed and the force line and weight should be adjusted at any time.

(2) Neck protection: A simple neck brace can limit excessive neck movement, increase the support of the neck and reduce the pressure in the intervertebral space. For patients with severe cases who need to get up and move around, a neck brace with traction can be used. For patients whose symptoms are relieved after neck traction and those in the recovery period after surgery, a neck brace is also needed to facilitate recovery.

(3) Physical therapy and massage: Among the commonly used physical therapy methods, wax therapy and acetic acid ion perfusion are more effective and can be used for mild cases. When choosing massage therapy, you should pay attention that although manual manipulation is effective for some cases, it may aggravate the symptoms or even cause paralysis if it is not performed properly or there are special pathological changes. Therefore, you must be cautious when deciding to use it.

(4) Drug treatment: Anti-inflammatory and analgesic drugs, such as diclofenac (Voltaren), diclofenac sodium/misoprostol (Osmotec), etc., can be used appropriately to alleviate the condition. In addition, compound danshen preparations have the effect of promoting blood circulation and removing blood stasis, and can also be taken orally. For those with obvious symptoms, intravenous drip can be selected, which is more effective than oral administration.

The cervical spine is one of the most important parts of our body.
