The symptoms of nail melanoma must be taken seriously. Terminal maculoid melanoma mainly occurs on the palms, soles and subungual areas. The lesions in the radiation growth phase are brown, brown or black and do not protrude above the skin surface. If irregular brown or brown stripes can be seen under the nail, they extend from the nail bed to the proximal end. The radiation growth phase lasts for about 1 year. If not treated in time, it will enter the vertical growth phase. The lesions are nodular and the lymph node metastasis rate also increases, and the prognosis is also poor.

The differential diagnosis includes basal cell carcinoma, seborrheic keratosis, dysplastic nevus, blue nevus, dermatofibroma, various nevus types, hemangioma (especially of the hands and feet), venous lakes, pyogenic granulomas, and warts. If in doubt, a full-thickness skin biopsy slightly beyond the lesion must be performed. Excisional biopsy is done for small lesions, and incisional biopsy is done for larger lesions. Serial biopsy sections can determine the depth of melanoma invasion, and thorough radical surgery must be performed only after a clear histological diagnosis.
