Causes

Amyloid is an amorphous, glassy, ​​transparent substance that contains AL protein (amyloid protein), AA protein (amyloid protein A), prealbumin, peptide proteins, Bz microglobulin, etc. Macrophages are considered to be the main site for the formation and degradation of amyloid in the body. When the production of amyloid exceeds the ability of macrophages to metabolize protein, amyloid is deposited in tissues. Amyloidosis is divided into: 1. Primary (or idiopathic).

2. Secondary (1) neoplastic diseases: 1) multiple myeloma. 2) Hodgkin's disease. 3) Medullary thyroid tumor. 4) Leukemia. (2) Infectious diseases: 1) Chronic suppurative infection. 2) Tuberculosis. 3) Syphilis. (3) Immune diseases: rheumatoid arthritis. (4) Hereditary amyloidosis. About 3/4 of patients with secondary amyloidosis develop amyloid nephropathy, while only 1/4 of patients with primary amyloidosis have amyloid nephropathy.

Treatment 1. Treatment of the underlying disease. 2. The efficacy of cytotoxic drugs in the treatment of amyloid nephropathy is still uncertain. Phenylalanine mustard (Melphalan) can significantly reduce proteinuria in the treatment of amyloidosis-nephropathy caused by multiple myeloma. 3. Colchicine Colchicine is recommended for the treatment of amyloidosis caused by familial Mediterranean fever, 1 to 1. 5mg/d can reduce the onset of amyloidosis and relieve symptoms. Hemodialysis is used for renal failure caused by amyloid nephropathy. Prognosis Compared with other glomerular diseases, the prognosis of this disease is poor, and heart failure, arrhythmia, sudden death and renal failure are the main causes of death.
