Binswanger's disease, also known as subcortical arteriosclerotic encephalopathy, is a relatively common small vessel dementia with a significant history of hypertension. Middle-aged and elderly patients may experience symptoms such as cognitive dysfunction, mild limb movement disorders, ataxia and urinary incontinence. Neuroimaging shows white matter atrophy, periventricular white matter loosening and multiple lacunar infarctions. Early detection of the disease can avoid the risk of stroke.

The cause of Binswanger's disease is unclear. Fisher (1989) found that 94% of 72 patients with pathologically confirmed Binswanger's disease had a history of hypertension and suggested that the disease may be related to hypertension and hyalinization of deep arterioles in the white matter.

1. The disease usually occurs between the ages of 55 and 65. Most cases have a history of hypertension for many years. The onset is insidious and the course is subacute or chronic.

2. Manifestations include chronic progressive dementia, focal neurological localization signs and psychiatric symptoms; the condition may be stable for a long time or rapidly worsen after a stroke. The first symptom is often cognitive impairment, memory loss, depression, and disorientation, which progresses to the inability to take care of oneself. Limb movement disorders are mild, and symptoms such as ataxia and urinary incontinence may occur. Complete hemiplegia signs rarely appear, and pseudobulbar palsy may occur.
