Overview

Traction therapy is a method of applying external force to a certain part of the body or joint to cause a certain separation and proper stretching of the surrounding soft tissue, thereby achieving the purpose of treatment. Spinal traction therapy usually refers to the use of external force to pull the lumbar spine-pelvis to achieve therapeutic purposes, which is called lumbar traction.

Generally, the supine position with hip and knee flexion is adopted to minimize the stress on the spine. The traction force usually starts with half of the patient's body weight and is gradually increased according to the situation, up to a maximum of the patient's body weight. Intermittent traction is the main method, with each traction lasting 20-30 minutes, 1-2 times a day, and a course of treatment of 15-20 days.

According to the size of traction and the duration of action, traction can be divided into rapid traction and slow traction. Rapid traction has a large weight and a short action time, and is often accompanied by the fingertips of traditional Chinese medicine. This type of traction is most typically based on the "artificial pulling and pressing reduction" method of traditional Chinese medicine. Later, it gradually developed into rapid horizontal traction with mechanical transmission. In recent years, researchers have combined the inclined board and rotation techniques of traditional Chinese medicine with the rapid horizontal traction of mechanical transmission to create an angled rotating rapid traction bed, which has been used in clinical practice to relieve the pain of many patients with lumbar disc herniation. Slow traction has a small weight and each traction lasts from 30 minutes to 1 hour. Multiple tractions are required and it is also a commonly used method for clinical treatment of lumbar disc herniation.

1. Improve the relationship between the protrusion and the nerve: For patients with mild or early lumbar disc herniation, traction therapy can gradually open the intervertebral space, which is conducive to the reduction of the protrusion. For patients with relatively long course of illness, traction can pull apart adhesion tissues and contracted ligaments and joint capsules to widen the spinal canal space accordingly. The narrow intervertebral foramina on both sides can also be pulled apart at the same time, thereby relieving or eliminating compression and stimulation of the nerve roots, and has a good effect in relieving numbness and pain in the lower limbs.

2. It plays a role in fixing and braking the waist: during traction, under the balance of action force and reaction force, the pulled waist is in a relatively fixed normal alignment state, and the range and amplitude of waist movement are further limited than when resting in bed or wearing a waist belt, so as to reduce or eliminate local inflammatory reactions such as congestion, exudation, and edema.

3. Relax the waist and back muscles: Lumbar disc herniation is caused by compression or stimulation of the spinal nerves, often accompanied by waist and back muscle spasms, which not only leads to pain symptoms in the waist, but also causes lumbar malalignment. Traction therapy can gradually relax the back muscles and relieve muscle spasms.

4. Restore the normal alignment of the lumbar spine: During traction, if the patient's lumbar spine is placed in a physiological curve, the malalignment can gradually return to normal as the traction time increases.
