Binswanger's disease, also known as subcortical arteriosclerotic encephalopathy, is a relatively common small vessel dementia with a significant history of hypertension. Middle-aged and elderly patients may experience symptoms such as cognitive dysfunction, mild limb movement disorders, ataxia and urinary incontinence. Neuroimaging shows white matter atrophy, periventricular white matter loosening and multiple lacunar infarctions. Early detection of the disease can avoid the risk of stroke.

Treatment of Binswanger disease

1. Traditional Chinese Medicine Treatment

The selected ingredients include Panax notoginseng total saponins (Xuethiontong), puerarin (Pulalin) and ligustrazine (methylpyrazine), which have the effects of promoting blood circulation and removing blood stasis, improving blood viscosity and anti-platelet aggregation.

2. Western medicine treatment

(1) Improve cerebral circulation, increase cerebral blood flow, and improve oxygen utilization. ① Dihydroergoline eliminates vasospasm and increases blood flow, improving neuronal function. Commonly used drugs include dihydroergoline, which is taken orally, and nicergoline (ergoline bromonic acid ester). ② Calcium ion blockers increase cerebral blood flow, prevent calcium overload and free radical damage, such as dihydropyridines such as nimodipine; treat patients with leukoaraiosis accompanied by cognitive impairment, and the condition stabilizes or improves after 1 year, such as diphenylamines such as flunarizine. ③Niacin can increase cerebral blood flow and improve memory.

(2) Brain metabolic agents promote the utilization of amino acids, phospholipids and glucose by brain cells, enhance the patient's responsiveness and excitability, and improve memory. ① Pyrrolidone commonly used is piracetam (NovoRacetam) and aniracetam, which can increase the formation and operation of adenosine triphosphate (ATP) in the brain, increase glucose utilization and protein synthesis, and promote information transmission between the cerebral hemispheres. ②Meclofenoxate can act as a central hormone, increase glucose utilization, excite the central nervous system and improve learning and memory functions. ③ Dihydroergoline mesylate (dihydroergoline) enhances the release of neurotransmitters from presynaptic nerve endings, stimulates postsynaptic receptors, and improves nerve function and energy balance of brain cells. ④ Almitrine, such as Almitrine/Rovaxin (Ducoxib), can increase arterial oxygen partial pressure and blood oxygen saturation, increase oxygen supply, and improve microcirculation and brain metabolism. ⑤Others such as brain protein hydrolysate (cerebrolysin), citicoline (cytidine diphosphate choline), adenosine triphosphate (ATP), coenzyme A, etc.

(3) Brain protective drugs ① Calcium ion antagonists such as nimodipine and flunarizine. ② Excitatory amino acid receptor antagonists such as magnesium sulfate and MK801. ③ Free radical scavengers such as vitamin E, vitamin C and ginkgo leaf preparations.

Things to note when preventing Binswanger's disease:

1. Early detection and avoidance of risk factors for stroke, such as hypertension, diabetes, and hyperlipidemia, and active treatment. People with severe carotid artery stenosis can undergo surgical treatment, which helps reduce the incidence of vascular dementia.

2. Quit smoking, control drinking and eat a healthy diet.

3. Those with a clear genetic background should undergo genetic diagnosis and treatment.
