Clinical manifestations include skin erythema, urticaria, conjunctival and nasal mucosal congestion, angioedema, pharyngeal edema, bronchospasm and anaphylactic shock. For those who have allergic constitution or suspected allergy to ester local anesthetics, amide local anesthetics can be used. However, care should be taken to differentiate it from the toxic reaction of local anesthetics or the adverse reaction to epinephrine added to the local anesthetic solution.

The latter may cause the patient to have pale complexion, restlessness, palpitations, shortness of breath, tachycardia and increased blood pressure. This adverse reaction often occurs in people with hypertension, heart disease, emotional stress and the elderly, especially in cervical plexus block. If an allergic reaction to the local anesthetic occurs, the local anesthetic should be stopped immediately and the antihistamine drug diphenhydramine 10-50 mg should be injected intramuscularly and flumethasone 10-20 mg should be injected intravenously, while inhaling oxygen. In severe cases, vasoactive drugs can be used to maintain hemodynamic stability.
