Some ancient books also discuss fat tongue together with swollen tongue. In fact, big tongue is caused by deficiency of spleen yang qi, or dampness and cold, which leads to fat, loose, soft and light-colored tongue, and frequent tooth marks. It is a sign of deficiency and should be distinguished from swollen tongue.

Definition of fat tongue

Fat tongue: The tongue is fat and enlarged, or has teeth marks on the edges, and is light and tender in color. It is called a fat tongue.

Main symptoms of enlarged tongue

1. Spleen deficiency: Symptoms include a fat, tender and pale tongue. There are teeth marks on the edges of the tongue, the tongue coating is thin white, the face is pale and cold, there is shortness of breath and laziness to speak, fatigue and indigestion, abdominal distension and watery stools, and a weak or slow pulse. It is mostly due to the obstruction of water, dampness and phlegm, which causes water to be retained in the tongue and causes it to become enlarged. If the tongue is pale, plump and tender, and the body is slippery, it is due to spleen and kidney yang deficiency, inability of qi to transform fluid, and the upward flow of fluid. The treatment should be to strengthen the spleen and replenish qi. The prescription should be Balizhong Decoction or Buzhong Yiqi Decoction.

Treatment should be warming yang and removing dampness, such as Jinkui Shenqi Pills and Zhenwu Decoction. In addition, if the tongue is light red and fat, with yellow greasy coating, it is mostly due to the spleen and stomach damp-heat fighting with phlegm turbidity, and the overflow of cold dampness and phlegm turbidity. The treatment should be to eliminate dampness and expel phlegm, and it is suitable to use Artemisia capillaris, Akebia trifoliata, Feldspar, Gypsum and Gardenia jasminoides to clear away heat and dampness.

Clinical Medicine and Scientific Research on Enlarged Tongue

Obese tongue and tongue with tooth marks are mostly caused by the enlarged tongue being compressed by the edge of the teeth. Therefore, tongue with tooth marks is often seen together with obese tongue. It is mostly caused by spleen deficiency and is the main symptom of deficiency syndrome. If the tongue is pale and moist, it indicates spleen deficiency and excessive dampness and coldness.

The cause of tongue with tooth marks is mostly due to spleen deficiency and inability to absorb water, which causes moisture to block the tongue and make it swollen, which is squeezed by the teeth and forms tooth marks. Therefore, tooth marks are often seen together with a fat and tender tongue. On the one hand, the tongue is swollen, which is due to spleen yang deficiency and excessive moisture. On the other hand, the tongue muscles are weak and the mouth cannot be opened enough, which is due to spleen qi deficiency.
