Pathological etiology

The cause of bladder neck obstruction in women is not very clear, and it is currently believed that the main cause is chronic urinary tract infection. Urinary tract infection is related to the anatomical characteristics of the female urethra and endocrine disorders.

The female urethra is short and straight, with a wide urethral opening close to the vagina, making it susceptible to infection. Due to recurrent urethral infections, long-term chronic inflammation of the bladder neck, mucosal congestion and hypertrophy, submucosal cell infiltration, and hyperplasia of the smooth muscle and connective tissue of the bladder neck, the female paraurethral glands have multiple branches, with more than 30 glandular ducts surrounding the urethra and opening into the urethral cavity. The proximal glandular ducts are very short, groove-shaped or pit-shaped, and are susceptible to infection, hyperplasia, and cystic changes, causing bladder neck obstruction. Endocrine disorders are the main cause of urinary tract infections.
