(1) Immunosuppressants

1) Methotrexate (MTX) is an anti-folate anti-tumor drug and is one of the earliest cytotoxic drugs used to treat psoriasis. It is effective for all types of psoriasis. It is suitable for arthritis type, erythrodermic type, pustular type and generalized psoriasis vulgaris. Methotrexate can be used for those who are not effective with other therapies. It mainly acts on cell DNA synthesis, preventing DNA production and inhibiting cell nuclear mitosis. There are different opinions on the method of medication. Currently, it is generally 15 mg/week, and the maximum dose is 50 mg/week. It can be administered orally, intramuscularly or intravenously. This drug has a severe toxic effect on the liver. It can cause extensive fibrosis and cirrhosis of the liver. It has an inhibitory effect on the hematopoietic system, mainly manifested by a decrease in white blood cells. It also affects platelets and can cause a decrease in total blood count in severe cases. Therefore, liver function and blood routine tests should be checked regularly during medication, and the indications should be strictly selected. This drug is contraindicated for patients with liver and kidney dysfunction, anemia, leukopenia and pregnant women.

2) Ethylenimine is also known as imine 154, or diketoazine. It mainly inhibits the synthesis of deoxyribonucleic acid in cells, thereby inhibiting cell growth. Usage: Adults generally take 300-400 mg orally 2-3 times a day. Generally, significant effect is seen after 3-4 weeks of medication, with a significant efficacy of 58%-88%. Its main side effect is bone marrow suppression, resulting in a decrease in white blood cells and platelets. In recent years, there have been reports of leukemia occurring after using this drug, so blood routine checks should be performed regularly when using this drug. In order to reduce the side effects of leukopenia, drugs that increase white blood cells such as shark liver alcohol and leukocytes can be given as an auxiliary. The disadvantage of this drug is that it is easy to relapse after discontinuation of the drug. To avoid relapse, a small dose can be used for a longer period of time after the rash subsides. The daily dose of diketoacil can be reduced to 100 mg, or even 100 mg every other day. The relapse rate can be greatly reduced. If the disease relapses after discontinuation of diketoacil, the effect of using diketoacil again is often less than the initial application, and the dose used needs to be larger than the initial application.
