Fibrinous pericarditis is a relatively rare disease. Its main symptoms are chest pain and fever, accompanied by some endothelial cell exudation. Therefore, if you encounter this disease in your life, you should go to the hospital for treatment immediately.

Fibrinous pericarditis: In the acute phase, fibrin exudates, accompanied by infiltration of inflammatory cells and a small amount of endothelial cells, with no obvious fluid exudate, commonly known as "dry pericarditis". Symptoms of fibrinous pericarditis: ① Chest pain, sharp pain, also squeezing pain, located in the precordial area, related to breathing and body position; ② Fever; ③ Typical signs: pericardial friction sound.

1. Symptoms

Precordial pain is the main symptom, such as acute nonspecific neopericarditis and infectious pericarditis; the pain symptoms of slowly developing tuberculous or tumor pericarditis may not be obvious. The pain may be sharp and related to respiratory movements, often aggravated by coughing, deep breathing or changing body position; it is located in the precordial area and may radiate to the neck, left shoulder, left arm and left shoulder blade, and may also reach the upper abdomen; the pain may also be squeezing-like and located behind the sternum. The precordial pain caused by this disease may be similar to the pain of myocardial infarction, so care should be taken to differentiate it.

(II) Physical signs

Pericardial friction sound is a typical sign of fibrinous carditis. It is caused by the friction between the parietal layer and the visceral layer, which become rough due to inflammation, when the heart is moving. It presents a rough scratching sound and has nothing to do with the occurrence of heart sounds. It often covers the heart sounds and is closer to the ear than heart sounds. Typical friction sounds can be heard in three components: atrial contraction, ventricular contraction, and ventricular relaxation. However, most of them are only biphasic friction sounds that are roughly consistent with atrial contraction and relaxation. They are mostly located in the precordial area, most obvious between the 3rd and 4th ribs on the left side of the sternum. They are easier to hear when leaning forward in a sitting position, taking a deep breath, or applying pressure to the chest with a stethoscope. Pericardial friction sound may last for several hours or for several days or weeks; when the effusion increases and separates the two layers of pericardium, the friction sound disappears, but it can still be heard if there is partial pericardial adhesion. The diagnosis of carditis can be made by hearing a pericardial friction sound in the precordial area.
