1. Treatment principles

(2) Clarify the clinical type and determine whether there are any complications. Clear clinical classification is extremely important for correctly guiding treatment.

(3) Determining whether the patient is resistant to penicillin, tetracycline, etc. will help guide treatment correctly.

(4) Clarify whether there is concurrent chlamydia or mycoplasma infection. If there is concurrent chlamydia or mycoplasma infection, a combined drug treatment plan should be formulated.

(5) Correct, adequate, regular, and comprehensive treatment should select the drug that is most sensitive to gonococci. The dosage should be sufficient, the course of treatment should be regular, and the method of medication should be correct.

(6) Strictly evaluate the efficacy and conduct follow-up observations. The cure standard should be strictly followed and the efficacy evaluation should be adhered to. Only when the cure criteria are met can it be considered cured to prevent recurrence. Those who are cured should insist on regular check-ups.

(7) Examine and treat their sexual partners at the same time. The patient’s spouse or sexual partners should be examined and treated at the same time.

2. General Notes

No sexual activity until the disease is cured. Pay attention to rest. Those with complications must maintain the balance of water, electrolytes and carbohydrates. Pay attention to local hygiene of the genitals.

3. Systemic therapy

For uncomplicated gonorrhea, such as gonococcal urethritis, cervicitis, and proctitis, give ceftriaxone, IM, as a single dose; or spectinomycin, IM, as a single dose; or ceftriaxone, IM, as a single dose. The second choice is other third-generation cephalosporins, which can also be used as alternative drugs if they have been proven to be effective. If Chlamydia trachomatis infection cannot be ruled out, add anti-Chlamydia trachomatis drugs.

According to the data of gonococcal resistance monitoring in my country in recent years, chromosomal resistance of gonococcal isolates to penicillin and tetracycline is relatively common in my country. Penicillins and tetracyclines are currently not recommended drugs for the treatment of gonorrhea. In addition, fluoroquinolone-resistant gonococci are now relatively common in my country, and the rate of resistant strains is increasing year by year. In some areas, the resistance rate of gonococcal isolates to this type of drug is as high as 75% to 99%. In clinical practice, cases of failure of quinolone drugs to treat gonorrhea are also common. Therefore, fluoroquinolones are not recommended for the treatment of gonorrhea.

Gonorrhea in children: Children weighing more than 45 kg are treated according to the adult regimen, and children weighing less than 45 kg are treated according to the following regimen. Tetracyclines are contraindicated in children under 8 years of age. It is recommended to use ceftriaxone, intramuscular injection, single dose; or spectinomycin, intramuscular injection, single dose, the dosage is determined by the doctor.

Everyone should learn more about the treatment of gonorrhea. If gonorrhea is not treated in time, it will cause many complications, the most common of which are prostatitis and seminal vesiculitis. Women may also suffer from other gynecological diseases. It is necessary to seek treatment in time and understand some treatment principles. During the treatment, the sexual partner should also be able to be examined.
