What is Central Nervous System Leukemia

Central nervous system leukemia (CNS-L) is a clinical symptom and sign caused by the extramedullary infiltration of leukemia cells into the arachnoid membrane or the nerve tissue adjacent to the arachnoid membrane. It is a common complication of leukemia and has an important impact on prognosis. CNS-L is mainly seen in acute leukemia (AL), and rarely occurs in patients with chronic leukemia.

Chronic lymphocytic leukemia is even more rare, called CNS-L. AL is more common in acute lymphoblastic leukemia (ALL) and M4 and M5 types in acute myeloid leukemia (AML). Most patients with hyperleukocytosis (white blood cell count>100×109/L) will develop CNS-L during the course of the disease, and the incidence of T-cell ALL is also very high. The incidence of childhood ALL complicated by CNS-L is much higher than that of adult ALL.

Symptoms and signs

In about half of the patients, CNS-L develops within 1 to 5 months after the diagnosis of AL, 81.5% occur before complete remission or during relapse, and only 18.3% occur during complete remission. In a small number of patients, CNS-L is the first manifestation of leukemia and is easily misdiagnosed as other central nervous system diseases. Among the 118 cases of CNS-L in the Peking Union Medical College Hospital mentioned above, 11.5% of the cases had CNS-L as the initial clinical manifestation of leukemia.
