The tuberous roots of Aconitum contain 0.70-1.3% total biota. The total bioalkaloids in the tuberous roots of Aconitum are 0.82-1.56%. Both Aconitum and Aconitum contain highly toxic diester bioalkaloids: mesaconitine, hypoaconitine and aconitine, generally with mesaconitine or hypoaconitine as the main components. Recently, it has been proved that Aconitum carmichaelii also contains Aconitum carmichaelii alkaloid, which is a new alkaloid.

2. Pharmacological effects of raw Aconitum

The herbal preparation of Aconitum kusnezoffii, aconitine and hypaconitine all have analgesic effects on pain reactions induced by electrical stimulation of the rat tail or the hot plate method. The total bioalkaloids of Aconitum have anti-inflammatory effects on rabbit ear swelling caused by heat and experimental peritonitis in mice. Clinically, Aconitum kusnezoffii is made into an injection containing 2 mg of total bioalkali per 2 ml, which is injected intramuscularly or used in combination with other drugs to treat rheumatoid arthritis, low back pain, neuralgia, etc. Aconitine stimulates the sensory nerve endings of the skin and mucous membranes, causing itching and heat, and then inhibits them to produce a local anesthetic effect.

Aconitum is an important medicine for dispelling cold and relieving pain. It can not only remove the coldness in the meridians, but also disperse the coldness in the internal organs. However, it is highly toxic and should be used with caution.

3.2 Indications

Aconitum can disperse the cold in the meridians and relieve pain. It is suitable for rheumatism, rheumatoid arthritis, etc. For example, Aconitum Decoction can treat rheumatism, and it can disperse the cold in the internal organs and relieve pain. It is suitable for heart and abdominal pain caused by cold evil. For example, Aconitum Red Shizhi Pills in Chapter 9 can treat heart pain, Red Pills in Chapter 10 can treat abdominal fullness and pain, Da Aconitum Decoction and Aconitum Cinnamon Twig Decoction can treat abdominal pain caused by cold hernia.

Side effects of raw aconite

1. The dosage is too large . Aconitum is divided into Chuanwu and Caowu. Aconitum is more toxic. The poisoning dose of Aconitum varies greatly due to the different collection time, processing and decoction time. Symptoms of Aconite poisoning include numbness of the mouth, tongue and whole body, nausea and vomiting, chest tightness, convulsions, difficulty breathing, decreased blood pressure, no increase in body temperature, heart rhythm disorder, confusion, coma, and even death from circulatory and respiratory failure. The symptoms of poisoning by Aconitum kusnezoffii are basically the same as those by Aconitum carmichaelii The amount of poisoning caused by aconite varies greatly from person to person. Some people are poisoned by taking only 6 grams of Chuanwu, while others are poisoned by taking only 1 gram of Caowu. 2. Unprocessed Aconitum is highly toxic, so it must be processed before oral administration. The prescription for oral administration should also indicate processed Aconitum, processed Chuanwu, or processed Caowu. If taken without being processed, even a small amount can cause poisoning. 3. Improper compatibility. Currently, people believe that Aconitum should not be used together with Fritillaria, Bletilla striata, Pinellia, and Trichosanthes. In addition, there are clinical reports of poisoning caused by the combined use of Aconitum and Ephedra. 4. The decoction time is too short. Long-term decoction of Aconitum or the recently adopted high-pressure steaming can reduce its toxicity. If the condition requires increasing the dosage of Aconitum, it is better to decoct it for a longer time. Decoction time that is too short can easily lead to poisoning. Aconitum should never be used raw.
