First, is trimalleolar fracture serious? Trimalleolar fracture refers to fractures or bone cracks of varying degrees occurring simultaneously at the medial malleolus, lateral malleolus, and posterior malleolus. The medial malleolus is the distal end of the tibia, and the lateral malleolus is the distal end of the fibula. The posterior malleolus, also called the posterior lip, is the posterior edge of the articular surface of the tibia and talus. For patients with non-open ankle injuries and obvious tibiotalar dislocation, manual reduction must be performed as soon as possible before surgery; for open injuries, thorough debridement is required. First, make an arc-shaped incision on the posterior medial malleolus to expose the fracture of the posterior medial malleolus. When exposing the posterior malleolus, be careful to cut the posterior tibial muscle sheath and push it backwards to avoid damaging the posterior tibial vessels and tibial nerve. The medial and posterior malleolus was reduced, temporarily fixed with a Kirschner wire, and then fixed with a lag screw. If the tibial articular surface collapses, the articular surface must be restored to its flatness and cancellous bone grafting must be performed if necessary. An outer incision was made at the lateral malleolus to expose the fibula and the distal tibiofibular syndesmosis. After the fibula was reduced, internal fixation with plates and screws was performed.

Second, if combined with tibiofibular separation, the anatomical relationship of the tibiofibular syndesmosis must be restored, and a 4.5mm lag screw should be used to penetrate the tibia and fibula parallel to the ankle joint surface 2 to 3 cm above the ankle joint surface for fixation. Check the stability of the fold, flush the incision and ankle, and suture the two incisions in layers. A film drainage was placed on the medial malleolus and removed after 36 hours. Antibiotics are routinely used for 5 to 7 days after surgery. Active ankle flexion and extension exercises are started 2 to 3 days after surgery, and weight-bearing walking is gradually allowed only when the fracture line is blurred on X-ray review. The functional position of the lower leg was maintained with plaster splint external fixation for 2 weeks after surgery.

Is trimalleolar fracture serious? Of course trimalleolar fracture is serious. It will completely destroy the normal function of the ankle joint and requires surgical reduction and internal fixation. Even after treatment, it may cause wear and tear of the ankle joint and bone hyperplasia, affecting the stability of the ankle joint.
