What is abdominal aortic aneurysm?

1. Pain

It is a common symptom before rupture, mostly located around the navel and upper and middle abdomen. When an aneurysm invades the lumbar spine, there may be pain in the lumbar sacral region. If severe abdominal or lower back pain occurs in the near future, it often indicates that the tumor is about to rupture.

2. Rupture

Patients with acute rupture present with sudden severe pain in the lower back and back, accompanied by shock, and may even die before admission to the hospital. If it ruptures into the retroperitoneum, bleeding is limited to form a hematoma. Abdominal pain and hemorrhagic shock may last for hours or days, but the hematoma often ruptures again into the peritoneal cavity and may cause death. The tumor can also rupture into the inferior vena cava, resulting in an aortovenous fistula, which may lead to heart failure. The tumor may occasionally rupture into the duodenum and cause massive gastrointestinal bleeding.

3. Other serious complications

Acute thrombus may occasionally form in the tumor, and the detachment of thrombus may cause embolism of the lower limb arteries. Compression of the duodenum may cause intestinal obstruction, and compression and obstruction of the inferior vena cava may cause peripheral edema.

5. Treatment

1. Medication

Measures such as controlling blood pressure, heart rate, blood lipids, and quitting smoking can control the increase in aneurysm diameter to a certain extent.

2. Surgery

Abdominal aortic aneurysm resection and artificial blood vessel replacement are still the classic surgical procedures for treating this disease. Indications for surgery include:

(1) The diameter of the abdominal aortic aneurysm is greater than or equal to 5.5 cm.

(2) During the follow-up period, the diameter increases by more than 1 cm each year.

(3) Symptomatic abdominal aortic aneurysm.
