Herpes zoster virus (VZV) is a type of herpes simplex virus that causes two different conditions. When children are infected for the first time, it can cause chickenpox. After recovery, the virus remains dormant in the body. When the latent virus is exposed to certain stimuli, it can relapse and cause shingles, which is more common in adults and the elderly, so it is called varicella-zoster virus.

VZV belongs to the genus Herpesvirus, and its biological characteristics are similar to those of herpes simplex virus (HSV), but it has only one serotype. Human fibroblasts and various monkey cells are commonly used to culture VZV. Typical cytopathic changes appear around 3-14 days, such as the formation of multinucleated giant cells (CPE) and the production of eosinophilic inclusion bodies in the nuclei of infected cells.

How herpes zoster virus is transmitted

Humans are the only natural host of varicella-zoster virus, and skin epithelial cells are the main target cells. The virus enters the body through droplets through the respiratory tract or contact infection. After two rounds of viremia, the virus replicates in large quantities and spreads throughout the body, especially the skin and mucous membranes. After an incubation period of about 2-3 weeks, papules, vesicles and pustules appear extensively on the skin of the body. The rash is distributed centripetally, with more cases on the trunk, and may develop into herpes.

The blister rash contains a large amount of viruses. No scars are left after the chickenpox disappears. The condition is generally mild, but occasionally there are complications such as interstitial pneumonia and post-infectious encephalitis. Children with cellular immunodeficiency, leukemia, kidney disease, or long-term use of corticosteroids or antimetabolites may develop severe or even life-threatening chickenpox. When adults suffer from chickenpox, 20%-30% develop pneumonia, which is generally severe and has a high mortality rate. The symptoms of chickenpox in pregnant women are also more serious and can cause fetal malformations, miscarriage or stillbirth.
