Generally, rubella in children and adults is mainly transmitted by droplets through the respiratory tract, and can also be transmitted between close contact between people. Newborns infected in the womb can excrete the virus, especially in the throat, for weeks, months or even more than a year. Therefore, they can infect medical staff and family members who lack antibodies through contaminated bottles, nipples, clothes, diapers, and direct contact, or cause transmission in the nursery. If a pregnant woman is infected with rubella in the early stages of her pregnancy, the rubella virus can infect the fetus through the placenta. Infection of the fetus can cause miscarriage, stillbirth, premature birth, or congenital rubella with multiple congenital malformations.

This disease is more common in children aged 5 to 9 years old. Children under 6 months old rarely get the disease because of passive immunity transmitted from the mother. Most people have lasting immunity after becoming ill. The disease occurs more frequently in winter and spring (spring and summer in recent years), and can be prevalent in kindergartens, schools, and the military.
