The presence and severity of symptoms of lower limb arteriosclerosis obliterans are affected by many factors, including the rate of disease progression, the amount of collateral circulation, and the individual's tolerance. Symptoms generally develop gradually from mild to severe, but when acute thrombosis occurs on the basis of arteriosclerosis obliterans, symptoms may suddenly worsen.

There may be no obvious symptoms in the early stages, or only mild discomfort, such as chills and fever. Afterwards, intermittent claudication symptoms gradually appear, which is a characteristic symptom of lower limb arteriosclerosis obliterans. The symptoms are fatigue and soreness in the affected limb after walking for a certain distance, and the patient is forced to rest for a period of time. The symptoms can be completely relieved after resting, but will reappear after walking again. The walking distance and rest time of each time are generally fixed. In addition, the location of soreness is correlated with the location of vascular lesions. As the lesion progresses, rest pain will occur, that is, pain in the extremities occurs when the patient is resting, which is more likely to occur when lying flat or resting at night. Eventually, ulcers and gangrene may develop in the limbs, often triggered by minor limb injuries.

The severity of clinical manifestations can be divided into Fontine staging or Rutherford staging to increase the objectivity of clinical evaluation and make the results of various clinical treatments more comparable. The Rutherford staging system is currently commonly used, which is divided into seven levels from 0 to 6 from mild to severe.
