In addition, immature leukocytes often extensively infiltrate the liver, spleen, lymph nodes, and other organs including the skin. Clinically, the specific lesions of various types of cutaneous leukemia are macules, papules, plaques, nodules, ecchymoses, and palpable purpura. The histological characteristics of most cutaneous leukemias are dense or diffuse infiltration of atypical leukocytes around blood vessels and glands, with a non-infiltrated zone under the epidermis.

The lesions are red or purple, slightly raised papules or nodules. In some cases, the papules may merge into plaques. Hemorrhagic lesions may occur. As the nodules continue to grow, they may form ulcers. The lesions are widespread, with the trunk, limbs, and neck being the most commonly affected areas. However, there are also cases where single large nodules or plaques occur.

Immature atypical myeloid cells and bizarre nuclei are common. Mitotic figures are common in AML infiltrates. The infiltrating cells of CML are more polymorphic, mainly granulocytes at different stages of differentiation, including atypical myeloid cells, metamyelocytes, and neutrophils. Granular cell sarcoma is composed of large mature cells mixed with immature myeloid cells that diffusely and confluently infiltrate the dermis and subcutaneous tissue.
