(A) Drug allergic stomatitis (drug-induced stomatitis): It is a type I allergic reaction and may also be a mixed type. Allergenic drugs are mostly caused by sulfonamides, antipyretics and analgesics (salicylic acid, aminopyrine, etc.), antibiotics, barbiturates, etc. There is a certain incubation period after the patient takes the medicine. The incubation period can be long or short. The disease may occur within 24 hours or even within a few minutes.

The main lesions are erythema, edema and blisters of varying sizes. The blisters rupture to form erosions or ulcers, causing swelling and pain. Some patients only have lesions in the oral cavity, while others have erythema, blisters and erosions on their genitals, conjunctivitis in the eyes, and small erythema, papules and blisters on the skin. In severe cases, exfoliative dermatitis or epidermal necrolysis may occur.

Another type of drug allergy is a fixed drug eruption, which usually occurs in the same area each time. It usually occurs at the junction of the skin and mucous membranes, such as around the mouth and limbs. It manifests as edematous erythema with blisters in the center and pigmentation left after healing. There is local burning sensation. Itch.

(ii) Contact stomatitis: It is mainly caused by type IV allergic reaction. Common allergens include methyl methacrylate, self-gelling acrylate, silver-mercury alloy, lipstick, bubble gum, etc. A reaction may occur if the person comes into contact with the allergen again after half a day or even more than 24 hours of contact with the allergen. The mucous membrane at the contact site will become red and edematous, blisters, erosions or ulcers may occur on the erythema, and there will be burning pain. Clinically, after silver-mercury alloy fillings are used, lesions similar to lichen planus can be seen on the contacted mucosa. If the silver-mercury fillings are removed and replaced with resin fillings, the lesions can disappear.
