Atrial flutter can be considered an intermediate form between atrial tachycardia and atrial fibrillation. When the frequency of the atrial ectopic pacemaker reaches 250 to 350 beats per minute and is regular, it causes rapid and coordinated contractions of the atria, which is called atrial flutter. Patients may experience hypotension, dizziness, palpitations, angina pectoris, and even cardiogenic shock.

1. Synchronous direct current cardioversion should be the first choice for patients with preexcitation syndrome and atrial flutter, or those with heart failure and cardiogenic shock who need emergency cardioversion and have 1:1 atrioventricular conduction and a ventricular rate of more than 200 beats/minute. Generally, 50 to 100 Ws is used, with a success rate of 100%.

2. If the ventricular rate is too fast, intravenous injection of sedilamide can be given, which can first turn into atrial fibrillation and then convert to sinus rhythm.

3. Amiodarone can be injected intravenously or taken orally, and can also achieve good therapeutic effects.

4. Propafenone (Pyridazone), quinidine, disopyramide (disopyramide), sotalol, flecainide, etc. can convert atrial flutter.

5. For some patients with chronic atrial flutter that cannot be converted by the above methods, oral digitalis or verapamil (Isapamil) (excluding heart failure) can be used to control it.

6. Catheter radiofrequency ablation has good treatment effects and can cure most patients.

Prognosis

Atrial fibrillation and atrial flutter mostly occur in coronary heart disease, hypertensive heart disease, cor pulmonale, hypokalemia, acute lung infection or digitalis poisoning. Therefore, the cause should be identified first and the underlying disease should be treated actively. Generally, if frequent premature atrial contractions occur before the onset of atrial fibrillation or atrial flutter, active treatment should be given to prevent them from developing into atrial fibrillation or atrial flutter.
