Gastroesophageal reflux disease can be treated initially with conservative medical treatment, including changing bad lifestyle habits and medication.

1. For patients with gastroesophageal reflux disease, do not lie down in bed immediately after eating during the day. To reduce lying position and nocturnal reflux, the head of the bed can be raised by 15-20 cm and eating should be avoided 2 hours before going to bed.

2. Pay attention to reducing all factors that affect increased abdominal pressure, such as obesity, constipation, tight waist belt, etc.;

3. Avoid eating foods (such as high fat, chocolate, coffee, strong tea, etc.) and taking medicines that reduce the tension of the lower esophageal sphincter. Elderly patients are prone to gastroesophageal reflux due to decreased function of the lower esophageal sphincter. If they also have cardiovascular disease and take nitroglycerin preparations or calcium channel blockers (such as nifedipine), reflux symptoms may be aggravated and should be avoided. If bronchial asthma is combined with gastroesophageal reflux, it may aggravate or induce asthma symptoms. Try to avoid the use of theophylline and dopamine receptor agonists (such as dopamine).

4. Drug treatment often uses a combination of acid suppressants and gastrointestinal prokinetic drugs. Acid-suppressing drugs include cimetidine, ranitidine, famotidine, etc., which can reduce 24-hour gastric acid secretion by 50%-70%, and the course of treatment is 8-12 weeks; proton pump inhibitors such as omeprazole and pantoprazole have stronger acid-suppressing effects, and the course of treatment is 4-8 weeks. The preferred gastrointestinal prokinetic drugs are cisapride and mosapride, and the course of treatment is 8-12 weeks. Gastroesophageal reflux disease has a tendency to chronically relapse, with a relapse rate as high as 70%-80% . In order to reduce relapse, the above-mentioned drugs often require long-term maintenance. For patients who are ineffective with drug treatment or develop severe complications, surgical treatment may be considered.
