Ascaris is a common parasite in the human intestine. It is often caused by accidentally eating food containing ascaris eggs, which is extremely harmful to human health. The pathogens will flow to various parts of the body through the blood circulation. If they flow to the internal organs, they will often induce a variety of symptoms, such as biliary ascariasis, intestinal obstruction, appendicitis, and even pancreatitis. Main clinical manifestations: 1. Biliary ascariasis: When the intestinal environment or the host's systemic condition changes, ascarids may be stimulated and burrow into the bile duct, causing biliary ascariasis. This disease is one of the main complications of intestinal ascariasis, second only to appendicitis, cholecystitis and perforated peritonitis. It is more common in adults and children, especially in young and middle-aged people, and more common in women than in men. 2. Ascaris ileus If there are more than ten intestinal roundworms, they can become tangled in the small intestine and cause mechanical intestinal obstruction. This disease is more common in severely infected children, more than 60% of whom are under 10 years old, with the highest incidence rate among those under 2 years old. Ascaris ileus is mostly incomplete intestinal obstruction, and the obstruction site is mostly in the lower ileum. Typical symptoms of ascariasis ileus include abdominal pain, vomiting, abdominal distension, cessation of bowel movements and flatulence, dehydration, acidosis, and electrolyte imbalance. 3. Ascaris appendicitis Improper deworming may cause ascaris to drill into the appendix and cause obstruction of the appendix cavity. Due to the drilling of the worm and the toxins it secretes, the appendix mucosa is stimulated, causing the appendix muscle layer and blood vessels to contract, blood supply to be blocked, resulting in mucosal damage and causing acute appendicitis. If the obstruction of the appendix cavity worsens progressively, the pressure in the cavity increases, which may lead to appendix perforation and secondary peritonitis. 4. Ascaris pancreatitis Ascaris invasion of the pancreatic duct can cause partial obstruction of the duct. Acute pancreatitis can be caused by the activation of pancreatic enzymes due to mechanical damage to the worm body, egg deposition and irritation, secondary bacterial infection, toxin action, and bile reflux. The symptoms of ascariasis pancreatitis are similar to those of general acute pancreatitis. Sudden onset of epigastric pain, nausea, and vomiting; followed by persistent, paroxysmal abdominal pain, chills, and fever; epigastric tenderness and high abdominal muscle tension 5. Ascaris liver disease In a small number of patients with biliary ascariasis, ascaris may enter the liver and bring in bacteria, leading to secondary infection and bacterial liver abscess. Abscesses are most common in the right lobe of the liver and less common in the left lobe. They can be single or multiple and vary in size. Ascaris and their eggs can be found in the pus; foreign body reactions caused by the eggs and bodies of the worms can be detected on the wall of the abscess. |
