Corneal ulcer is a common eye disease in clinical practice. This disease can cause great harm to people's eyes and directly affect people's lives. Generally speaking, when corneal ulcer occurs, treatment measures must be taken in time, which will affect people's vision in daily life. The causes of corneal ulcers are quite complicated, and their causes must also be considered during treatment. So how should corneal ulcers be treated? 1. Common treatment methods Eliminate the causes, promptly handle and treat eye diseases, and control infections by targeting pathogenic microorganisms, mydriasis, hot compresses, and the use of corticosteroids. Various vitamins, such as vitamins CE and AD, can be used for bandaging and supportive therapy. 2. Treatment of refractory corneal ulcer (1) Corneal ablation. (2) Cryotherapy: After surface anesthesia, fluorescent staining is used to determine the freezing range. (3) Application of collagenase inhibitors. (4) Surgery: small conjunctival flap surgery, therapeutic keratoplasty, and application of medical adhesives. 3. Treatment of corneal scars Use drugs to promote scar absorption and surgery. The surgery may be laser iridectomy, optical iridectomy or corneal transplantation, depending on the location, range, thickness and impact of the corneal scar on vision. Anti-glaucoma surgery can be performed for secondary glaucoma caused by adhesional leukoplakia. 4. Lamellar Keratoplasty It is a partial-thickness corneal transplant. During surgery, the diseased tissue in front of the cornea is removed, leaving the underlying tissue as a transplant bed. The graft bed is usually very thin, with only the Descemet's membrane and endothelium remaining. Therefore, lamellar keratoplasty can be performed on patients whose corneal lesions have not invaded the deep layer of the corneal stroma or the Descemet's membrane and whose endothelial physiological function is healthy or recoverable. It is commonly used clinically for medium and shallow corneal leucoma or corneal dystrophic opacity, progressive keratitis or ulcer, corneal fistula, corneal tumor, and some eyes with poor conditions that cannot undergo penetrating keratoplasty. In order to improve the corneal condition, lamellar transplantation is performed first. Currently, deep lamellar keratoplasty is commonly used, and deep lamellar keratoplasty can be performed with the assistance of femtosecond laser. |
