1. Cesarean section:

Cesarean section can quickly end labor and deliver the fetus in a short period of time. It is safer for both mother and child and is currently the main method of treating placenta previa.

Complete placenta previa must end delivery by cesarean section. In recent years, partial or marginal placenta previa in primiparas also tend to be delivered by cesarean section. Timely and decisive cesarean section can immediately end delivery and achieve the purpose of rapid bleeding. It reduces trauma to the fetus, reduces perinatal morbidity, and can treat postpartum hemorrhage under direct vision. It is the safest and most effective method for treating placenta previa, and is also the first aid method for treating severe bleeding from placenta previa.

Before surgery, shock should be actively corrected, and blood volume should be replenished through infusion and blood transfusion. These measures not only save the patient, but also improve the fetus's hypoxia in the uterus.

Cesarean section usually chooses the lower uterine incision. In principle, the placenta should be avoided, and the operation should be determined according to the location of placental attachment. B-mode ultrasound examination was performed before surgery to determine the location of placental attachment. If the placenta is attached to the posterior wall, a transverse incision is made in the lower segment; if the placenta is attached to the anterior wall, a longitudinal incision is made in the lower segment. If the placenta is attached to the uterine incision, the placenta should be pushed away to rupture the membranes. Due to the poor contraction force of the lower uterine segment, the placenta is not delivered immediately after the fetus is delivered, and manual separation must be performed in time. At the same time, 0.2-0.4 mg of ergonovine is injected into the uterine muscle wall to enhance the contraction of the lower uterine segment, combined with uterine massage, which can reduce postpartum bleeding.

2. Vaginal delivery:

It is only suitable for patients with marginal placenta previa, occipital presentation, little bleeding, and estimated delivery can be completed in a short time. After deciding on vaginal delivery, artificial rupture of membranes is performed. After rupture of membranes, the fetal head descends to compress the placenta to stop bleeding, and can promote uterine contraction and accelerate delivery. If the presenting part does not descend ideally after rupture of membranes, there is still bleeding, or the delivery is not progressing smoothly, a cesarean section should be performed immediately.

3. Treatment during emergency transfer: If the patient has heavy vaginal bleeding and there are no local conditions for treatment, intravenous infusion or blood transfusion can be given, and vaginal packing can be performed under disinfection to temporarily compress and stop the bleeding, and the patient should be quickly escorted to another hospital for treatment.
