The cerebellum is an important organ in the human body, but it is also a site prone to diseases. When the cerebellum is affected, the brain will also react abnormally. During a physical examination, one result may be a mass in the left cerebellopontine angle area, which many people do not fully understand. Because it is not clear whether it is a sign of illness, we must understand in detail what is the mass in the left cerebellopontine angle area?

Location:

The lesion was located in the left cerebellopontine angle and was closely connected to the left auditory nerve, highly suggestive of an acoustic neuroma. Meningiomas can be located in the cerebellopontine angle, and very rarely meningiomas can grow into the internal auditory canal, but the probability of this is reduced in this case. Trigeminal neuromas are mostly located in the anterior nerve course of the internal auditory canal. The auditory nerve is normal and unchanged, so the probability is reduced in this case. Epidermoid cysts can be located in the cerebellopontine angle and may compress and erode adjacent bones, but the probability of this is low in this case.

The left internal auditory canal was widened, and the lesion showed a typical ice cream sign, which was consistent with an acoustic neuroma. Meningiomas are often connected to the dura mater with a wide base, which reduces the probability. When epidermoid cysts are larger, they grow in a plastic manner, but the probability is lower in this case.

Enhancements:

Acoustic neuromas show moderate or above enhancement, and cystic changes are common. Meningioma has uniform signal and is often significantly enhanced, so the probability is reduced in this case. Trigeminal neuroma has moderate or above enhancement. If it is a fibroma, the signal is relatively uniform, while if it is a schwannoma, the signal is uneven. Epidermal cysts do not enhance.

DWI:

Epidermoid cysts are rich in large molecular protein substances, have restricted diffusion, show high signal on DWI and low signal on ADC, so they can be excluded in this case. Meningioma has dense cells and slightly restricted diffusion. DWI shows a slightly higher signal and ADC shows a slightly lower signal. Therefore, it can be ruled out if the signal does not match. Acoustic neuromas and trigeminal neuromas have unrestricted diffusion.
