1. Western Medicine

1. Treatment should be directed at the cause. During the acute phase of lumbar disc herniation, resting on a hard bed for 1-2 weeks can often stabilize the symptoms.

For symptomatic treatment, pain can be treated with paracetamol plus codeine 30 mg, 3-4 times/d, as well as other non-steroidal analgesics such as ibuprofen, naproxen, etc. Muscle spasms can be treated with diazepam 5-10 mg orally, 3 times a day; or cyclobenzaprine 10 mg orally, 3 times a day, which may be effective.

3. In severe cases, dexamethasone 10-15 mg/d can be used by intravenous drip for 7-10 days. Generally, prednisone 10 mg can be taken orally 3-4 times a day, and 10-14 times as a course of treatment. Paravertebral blockade can also be performed with 1%-2% procaine or 1 ml of prednisolone. It can be combined with acupuncture and physical therapy, and conservative treatments can often provide relief. When the therapeutic effect is not good, pelvic traction or prednisone epidural injection can be used. Surgical treatment can be considered for individual ineffective or chronic recurring cases.

Especially in the early stages of intervertebral disc herniation, some patients can rest on a hard bed for 3-4 weeks and their symptoms will improve on their own.

2. Drug treatment:

Analgesics, B vitamins, and a short course of oral corticosteroids can help recovery.

3. Physical therapy:

In the acute phase, ultrashort wave therapy, ultraviolet irradiation and other treatments can be used. In the chronic stage, shortwave therapy and direct current iodine ion introduction can be used.

[Composition] 30 grams of Sophora flavescens, 9 grams of Angelica dahurica, 9 grams of Aconitum carmichaelii, 6 grams of Cinnamon bark, 15 grams of Curcuma longa, 15 grams of Atractylodes macrocephala, 30 grams of Coix seed, and 9 grams of Stephania tetrandra.

[Usage] Decoction in water, 1 dose per day, twice a day.

[Composition] 15 grams of Angelica dahurica, 12 grams of Polygonum multiflorum, 10 grams of Rhizoma Coptidis, 12 grams of Eucommia ulmoides, 15 grams of Achyranthes bidentata, 12 grams of Dipsacus asper, 10 grams of Papaya, 30 grams of Millettia reticulata, 9 grams of Safflower, 12 grams of Angelica sinensis, 9 grams of Ligusticum chuanxiong, and 10 grams of Pheretima.

[Usage] Decoction in water, 1 dose per day, twice a day.

[Composition] 10 grams of processed Arisaema, 10 grams of Angelica dahurica, 10 grams of Phellodendron chinense, 10 grams of Chuanxiong, 10 grams of Carthamus tinctorius, 10 grams of Notopterygium wilfordii, 25 grams of Clematidis, 15 grams of Atractylodes macrocephala, 15 grams of Peach Kernel, 15 grams of Stephania tetrandra, 15 grams of Corydalis yanhusuo, 15 grams of Angelica dahurica, 6 grams of Gentiana scabra, 12 grams of Shenqu, and 12 grams of cinnamon twig.

[Usage] Decoction in water, 1 dose per day, twice a day, 3 days as a course of treatment.

[Composition] 6-12 grams of asarum, 6-12 grams of processed Aconitum kusnezoffii, 6-12 grams of processed Aconitum carmichaelii, 15 grams of Ephedra, 20 grams of Achyranthes bidentata, 20 grams of papaya, and 10 grams of frankincense.

[Usage] Decoction in water, 1 dose per day, 2 times a day. Start with a small amount of Asarum and processed Chuanwuwu and increase the amount gradually.
