Selective proteinuria

Selective proteinuria

Many friends don’t know what selective urine protein is. In fact, selectivity of urine protein is mainly a type of nephrotic syndrome. If the main components of our urine are small molecular weight plasma proteins, it means that our glomerular filtration membrane has better selectivity. This type of proteinuria is called selective proteinuria. Otherwise, it is called fat-selective proteinuria. So what is the normal value of selective proteinuria?

In fact, selective proteinuria is mainly used as one of the means to check whether our kidney function is normal. Generally speaking, the normal value of selective urine protein is between 0.1 and 0.2. If the value of this selective proteinuria increases, it will be extremely harmful to our body.

Selective proteinuria

Urine protein selectivity refers to the state of glomerular filtration membrane filtering proteins of various molecular weights in plasma. A typical disease is nephrotic syndrome. Selective proteinuria: When glomerular lesions are relatively mild, only small and medium molecular weight proteins (mainly albumin and a small amount of small molecular weight proteins) are excreted from the urine, while large molecular weight proteins (such as IgA, IgG, etc.) are excreted less. This type of proteinuria is called selective proteinuria, and semi-quantitative values are mostly between +++ and +++++.

Definition: If the urine mainly contains small molecular weight plasma proteins, it means that the selectivity of the glomerular filtration membrane is good, and this proteinuria is called selective proteinuria. On the contrary, if the urine mainly contains large molecular weight proteins, it means that the selectivity of the glomerular filtration membrane is poor, and this proteinuria is called non-selective proteinuria.

Urine index

Normal value of selective proteinuria index: When SPI is between 0.1 and 0.2, it is moderately selective and is a normal clinical manifestation. Clinical significance of selective proteinuria index: Abnormal results: High selectivity is seen in minimal change nephropathy, which is sensitive to hormones and has a better prognosis. Membranous or membranoproliferative nephritis often manifests as low selectivity or non-selectivity, indicating that a large number of large molecular proteins pass through the glomerular filtration membrane, respond poorly to hormones, and have a poor prognosis.

People who need to be examined: patients suspected of having kidney disease.

symptom

1. Under normal eating and drinking conditions, the amount of protein in the urine is continuously higher than 150 mg/24 hours in adults and more than 300 mg/24 hours in adolescents, and the routine qualitative test is positive, which can be called clinical proteinuria.

2. The amount of original urine filtered by a normal person is maintained at 180 liters per day, but only about 1.5 liters are reabsorbed and secreted by the renal tubules and finally concentrated and discharged. It contains about 40 to 100 mg of protein, which cannot be detected using the urine protein qualitative method.

3. Not all proteinuria is considered pathological. There are two types of proteinuria: functional proteinuria and pathological proteinuria. Functional proteinuria, also known as physiological proteinuria, refers to temporary proteinuria that occurs in healthy people. It is more common in young people. Under the influence of various combined factors such as strenuous exercise, fever, high temperature, cold, mental stress, etc., the renal blood vessels spasm or congestion, causing the permeability of the glomerular filtration membrane to increase, causing a large amount of protein to "leak through".

4. When proteinuria occurs, foamy urine is usually present. The symptom of foamy urine is that there is a layer of fine foam floating on the surface of the urine, which does not disappear for a long time.

The normal value of selective proteinuria is generally between 0.1 and 0.2, which is a normal manifestation and phenomenon. If a large amount of protein appears in the urine, it means that the selectivity of the glomerular filtration membrane is poor, so you need to seek medical attention and treatment in time. Don't be afraid to see a doctor. Only timely treatment can control the disease.
