Tretiakoff (1919) found that clinical manifestations of PD occur when the number of substantia nigra neurons decreases by ≥50%. The neurobiochemical changes of PD are as follows: the loss of neurons in the substantia nigra compacta and locus coeruleus in the midbrain leads to a decrease in dopamine (DA) in the above-mentioned areas and their nerve terminals (PD clinical manifestations occur when DA decreases by ≥70%), while the effect of acetylcholine (ACH) that antagonizes the function of DA in the substantia nigra and striatum system is relatively hyperactive, resulting in an imbalance in the balance between DA and ACH.

Due to limb tremors and difficulty in moving, Parkinson's patients are afraid of being laughed at by colleagues or friends, so they unconsciously isolate themselves and stay away from their familiar life and work circles. Stay at home all day. In this way, the disease progresses, the symptoms worsen, and the mood becomes extremely bad. Many Parkinson's patients have varying degrees of depression and anxiety symptoms.

Gradual loss of body motor function: Parkinson's disease symptoms generally start on one side and then gradually spread to the contralateral limbs. The effectiveness of drug treatment also gradually decreases, and side effects become more and more obvious. In the middle and late stages, it will affect swallowing and vocalization, difficulty turning over at night, insomnia, etc. Severe patients may become bedridden in the late stages due to muscle contracture and joint stiffness.

The family burden gradually increases: As the condition of Parkinson's patients worsens, Parkinson's families will face increasingly heavy human and financial burdens. Patients gradually develop from occasionally needing care to needing one or even two dedicated people to take care of their basic lives.
