Vascular Parkinsonism (VP) is clinically manifested by movement disorders in both lower limbs. For patients without a history of acute stroke or neuroimaging changes, the clinical manifestations are similar to those of senile gait disorders. As the initial symptom, rest tremor was found in only 1 case (4%) of the VP group and 14 cases (47%) of the PD group, with a very significant difference (P<0.001). As the disease progressed, tremor developed in 4 VP patients (17%), of which 3 were static tremor, and tremor developed in 22 PD patients (73%), of which 20 were static tremor (P<0.01). There were significant differences in myotonic hypertonia, asymmetric myotonia, and dementia between VP and PD patients. In addition, more than half of VP patients present with pyramidal tract signs and pseudobulbar palsy.

The main clinical features of patients with VP confirmed by pathology are as follows: ① The onset may be acute or slow, or may be hidden; it may also be acute or subacute; ② There is usually no resting tremor; ③ Increased myotonic tension; ④ Asymmetric limb rigidity; ⑤ Slow movements; ⑥ Panic gait; ⑦ Stiff expression, resembling a "mask face"; ⑧ More than half of the patients have pyramidal tract signs and pseudobulbar palsy; ⑨ VP patients also have dementia; ⑩ VP patients may have varying degrees of spontaneous remission.
