Case of metrorrhagia and functional uterine bleeding

Li Moumou, female, 49 years old, married, cadre. First visit on June 15, 2009.

Chief complaint: Menstruation lasts for more than 20 days.

Initial diagnosis: The patient usually has irregular menstruation, with prolonged menstruation, increased menstrual flow, and no blood clots. This menstruation started more than 20 days ago, but it has not stopped yet. It is continuous and accompanied by headache.

The patient was treated in another hospital, and B-ultrasound showed a cystic mass in the uterine adnexal area, approximately 3.1cmx2.8cm in size.

Symptoms during consultation: continuous menstruation, increased menstrual volume, no blood clots, accompanied by dizziness, no abdominal pain,

No headache, pale red tongue, thin white fur, deep and fine pulse.

TCM diagnosis: metrorrhagia, syndrome of kidney yang deficiency and Chong and Ren meridian disorders.

Western medicine diagnosis: functional uterine bleeding.

This is because the patient is nearly 77 years old, her kidney qi is gradually declining, the essence is about to be exhausted, the kidney has lost its storage function, and the Chong and Ren meridians are out of control, so the menstrual period is prolonged, the amount of blood is heavy, and it is dripping. The tongue is pale red, the tongue coating is thin white, and the pulse is deep and fine, all of which are signs of kidney yang deficiency. The method should be to warm the kidney, consolidate the Chong, stop bleeding and regulate menstruation.

20g of cuttlefish bone, 30g of antler cream (pre-fried), 6g of licorice, 159g of mugwort, 30g of processed Polygonum multiflorum

Cuscuta 18g, Dipsacus asper 18g, Viscum album 20g, Jujube 10g, Eucommia ulmoides 20g, Agrimonia 30g

4 doses, decocted in water, 1 dose per day.

Second visit (June 19, 2009): Menstruation stopped after taking 2 doses of the first medicine. Then she took the remaining medicine and now her menstruation has stopped, but she still feels dizzy from time to time. The patient's long-term leakage has damaged the yin blood, causing yin blood deficiency, liver and kidney deficiency, and lack of nourishment to the clear orifices, so dizziness may be seen. Third visit (December 11, 2009): All symptoms have improved. The menstrual cycle has been basically normal for the past 6 months, with occasional prolonged periods. This time, the menstruation lasted for more than 10 days, was dark red, contained blood clots, and was heavy. This patient originally had kidney yang deficiency, Chong and Ren meridians damage, menstrual disorders, and continuous leakage. Long-term leakage leads to blood stasis, blood stasis, and blood stasis that is not removed, making it difficult for new blood to settle, so the menstruation is heavy and difficult to stop. The method should be to regulate Chong and Ren meridians and eliminate blood stasis. The prescription is Shixiao Powder with modifications.

After taking 2 doses of the medicine, menstruation stopped.

Note: The patient has kidney yang deficiency and Chong and Ren meridian injuries. The previous treatment aimed to warm the kidney and stop bleeding, and achieved satisfactory results. However, the disease has affected the collaterals for a long time and there is also blood stasis. The menstruation that did not stop during this visit was caused by blood stasis. The treatment is to tonify the kidney and assist yang, regulate Chong and Ren, and eliminate blood stasis. The prescription uses "hemostatic trinity" i.e. deer antler frost, oyster and donkey-hide gelatin, plus dragon bone and dandelion charcoal to astringe and stop bleeding. Dipsacus asper and Cuscuta australis can nourish the liver and kidneys and regulate Chong and Ren meridians. Panax notoginseng can invigorate blood circulation, eliminate blood stasis and stop bleeding. It can stop bleeding without leaving stasis. The medicine is consistent with the indications and the effect is immediate.
