What is a large-area cerebral infarction? How to distinguish it from cerebral hemorrhage?

Large-area cerebral infarction is caused by obstruction of the main cerebral artery. CT shows large low-density shadows, which are mostly distributed in the cerebral lobes or across cerebral lobes. The area of ​​brain tissue damage is large. Clinically, in addition to the general symptoms of cerebral infarction, there are also impaired consciousness and increased intracranial pressure.

Cerebral hemorrhage, commonly known as cerebral hemorrhage, is a type of "cerebral stroke" and is a common and serious brain complication in middle-aged and elderly patients with hypertension. Cerebral hemorrhage refers to bleeding caused by non-traumatic rupture of blood vessels in the brain parenchyma. The most common causes are hypertension, cerebral arteriosclerosis, intracranial vascular malformations, etc. It is often triggered by factors such as exertion and emotional excitement, so most of them occur suddenly during activities. Clinically, cerebral hemorrhage develops very quickly, and the main manifestations are damage to the nervous system such as impaired consciousness, hemiplegia of limbs, and aphasia.

When there is a large area of ​​cerebral infarction with prominent symptoms of impaired consciousness and increased intracranial pressure, it is very similar to cerebral hemorrhage, making diagnosis difficult. A CT scan of the head is necessary for diagnosis, but if CT is not possible, the following points can be used as the basis for differential diagnosis:

(1) Patients with large-area cerebral infarction often have a history of cerebral thrombosis, and often have prodromal symptoms such as weakness, numbness, and dizziness on one side of the body a few days or weeks before the onset of the disease.

(2) If the disease occurs during work, defecation, drinking, or excitement, there is a high possibility of cerebral hemorrhage. Traditionally, it is believed that cerebral infarction often occurs in a quiet state.

(3) More than 70% of patients with cerebral hemorrhage have a history of hypertension, and most of them have significantly elevated blood pressure at the beginning of the disease; large-area cerebral infarction often does not have similar phenomena.

(4) Patients with cerebral hemorrhage will have symptoms of increased intracranial pressure such as headache and vomiting at the onset of the disease; increased intracranial pressure in large-area cerebral infarction appears relatively late and often worsens progressively.

(5) Meningeal irritation signs are common in cerebral hemorrhage and appear earlier; large-area cerebral infarction is generally difficult to detect or appears later.
