This type is more common. There are symptoms of peripheral circulatory failure of infectious shock, with high fever, mental fatigue, thirst, sweating in the early stage; followed by gray complexion, cold and wet limbs, rapid pulse, rapid breathing, decreased blood pressure, small pulse compression, low and dull heart sounds,

Symptoms include irritability, delirium, decreased urine volume, etc. In the late stage, there will be cyanosis of the lips and nails, mottled skin with bruises, disappearance of blood pressure and pulse, anuria, and coma.

(II) Cerebral type (cerebral edema type, respiratory failure type)

It develops faster than the shock type and is mainly characterized by severe brain symptoms. In the early stages, there are symptoms of cerebral edema and increased intracranial pressure, such as pale complexion, headache, frequent vomiting, rapid breathing, normal or high blood pressure, confusion, drowsiness or agitation, delirium, and sudden convulsions. In the late stages, blood pressure rises or suddenly drops, pupils vary in size or are unequal in size, light reflex is slow or disappears, there are repeated convulsions, and a deepening coma. Respiratory failure is manifested by uneven breathing and irregular rhythm, with sighing-like breathing, slow or paused breathing, and even sudden respiratory arrest and death.

(III) Hybrid

The patient has symptoms of both circulatory failure and respiratory failure, and the prognosis is poor. Death is often caused by high fever, shock, convulsions, and respiratory failure.

In summer and autumn, if there is a history of eating unclean food or contact with patients with bacillary dysentery; there are the above-mentioned clinical manifestations; there is pus and blood in the stool or anal swab or enema sampling, and a large number of pus cells and red blood cells are found under the stool microscope, the diagnosis can be made. The diagnosis is based on the presence of Shigella dysenteriae in stool culture. The diagnosis of this disease requires the exclusion of acute food poisoning, the shock type requires the exclusion of other infectious shocks, and the cerebral type requires the exclusion of epidemic encephalitis B, etc.
