Traditional Chinese medicine uses a dialectical treatment method to treat tinnitus. 1: Kidney-tonifying and sperm-replenishing method1: Traditional Chinese medicine believes that the kidneys are closely related to the ears. The kidneys are the foundation of our constitution, storing essence and producing marrow. They connect to the brain and open up to the ears. The prescription can be used with modifications: 30g of cooked rehmannia, 20g of polygonatum, 20g of yam, 3g of pilose antler, 12g of tortoise shell, 3g of placenta, 12g of taro, 15g of wolfberry, 12g of Achyranthes bidentata, and 15g of Cuscuta australis. 2. Heat-clearing and phlegm-resolving method1: For tinnitus and deafness, traditional Chinese medicine advocates starting with the kidney and treating it based on the phlegm. Such patients often have sudden tinnitus, obesity, dizziness and distension, coughing and spitting, flank pain, excessive and sticky sputum, red tongue edges and tip, yellow and greasy tongue coating, stringy and slippery pulse, etc., all of which are signs of internal accumulation of phlegm and heat. Three: Diuresis and expelling fluid1: Phlegm and fluid are both pathological products of abnormal water metabolism, which remain in local parts of the body. Phlegm is hot and fluid is cold. Phlegm is invisible and fluid is substantial. If fluid remains in the body, the clear yang will not rise, which can cause tinnitus, dizziness, etc. Diuretic and anti-edema drugs can relieve edema in the vestibule and labyrinth of the inner ear, reduce nerve compression, and promote local lymphatic return. The prescription of Ling Gui Zhu Gan Tang and Wu Ling San in Treatise on Febrile Diseases is modified: Poria 15g, cinnamon twig 10g, Atractylodes macrocephala 12g, Alisma orientalis 6g, Leonurus japonicus 20g, Stephania tetrandra 6g, Astragalus 30g, Plantago asiatica 10g. 4. Dispelling wind and clearing heat1: Patients with new tinnitus are mostly caused by viral infection, with an acute onset and cold symptoms. Chinese medicine diagnosis shows that it is caused by wind-heat. Due to exogenous wind-heat, the meridians are attacked upward, and the clear orifices are blocked, causing tinnitus like cicadas and sudden hearing loss or blockage. It is often accompanied by fever, chills, headache, nasal congestion and runny nose, dry mouth, dry throat, general fatigue, red tongue, thin yellow tongue coating, and floating and rapid pulse. The treatment should be to dispel wind and clear heat. The prescription is Yinqiao Powder with modifications: Flos Lonicerae 10 g, Fructus Forsythiae 10 g, Leaf of Bamboo 6 g, Schizonepeta Tenuifolia 10 g, Lotus Root 6 g, Chrysanthemum 10 g, Periostracum Cicadae 6 g, Radix Puerariae 10 g, Scutellariae 10 g. Five: Liver-clearing and fire-reducing method1: Some tinnitus patients may be caused by pyogenic infection or have a history of hypertension. In addition, they are fond of spicy food, which may damage the liver due to anger and depression, and cause liver fire to rise up along the meridians. According to TCM diagnosis, it is excess heat and liver fire. The liver is the general's organ, with a strong nature. It is responsible for promoting and discharging. If the liver loses its order, it becomes depressed and turns into fire, disturbing the clear orifices, then tinnitus will occur, like tides and thunder, rumbling, often accompanied by ear swelling and pain, pus discharge, fever, headache and dizziness, flushed face and eyes, bitter mouth and dry throat, irritability, red tongue with yellow coating, and a stringy and strong pulse. The treatment should be to soothe the liver and clear the fire. The prescription is Longdan Xiegan Decoction, Tianma Uncaria Decoction, Zhengan Xifeng Decoction, etc. with modifications: Gentiana 9g, Gastrodia 12g, Uncaria 10g (added later), Rhizoma Cyathulae 12g, Scutellaria 10g, Chrysanthemum 12g, White Peony 12g, Rehmannia glutinosa 15g, Hematite 20g (decocted first), Malt 9g. VI: Blood Activating and Stasis Removing Method1: Tinnitus caused by long-term illness. Traditional Chinese medicine believes that "long-term illness is in the blood" and "long-term illness is mostly caused by blood stasis", which blocks the ear orifices, causing poor circulation of qi and blood and malnutrition of the ear orifices. The microcirculation changes in patients with blood stasis syndrome in TCM are more obvious. Some of these patients suffer from vasospasm and blood flow obstruction due to stimulation of the ear blood vessels. Western medicine such as 654-2, Sibelium, and Salvia miltiorrhiza injection can relieve spasms and restore blood flow. Clinically, this type of patients also have symptoms of blood stasis such as stabbing headache with a fixed location, dizziness, purple tongue, and thin and wiry pulse.
