1. Drug treatment

1. β-blockers: 40-120 mg of propranolol, divided into 2 doses, which need to be taken irregularly. If the tremor is expected to be particularly severe in a specific situation, 40-120 mg of propranolol can be taken orally temporarily;

Almar has better effects and fewer side effects and is more commonly used in clinical practice. The usual dose is 10 mg twice a day, taken in the morning and afternoon. An electrocardiogram should be performed before taking the medicine to rule out contraindications of beta-blockers such as slow heart rate and conduction block.

2. Primocide is also effective, but patients with essential tremor are often very sensitive to this drug and should not be used to treat epilepsy. It should be started with a small dose of 50 mg/d, and the daily dose should be increased by 50 mg every 2 weeks until it is effective or side effects occur; the usual effective dose is 100-150 mg, 3 times/d;

3. Sedatives can be selected from alprazolam 0.2-0.4 mg/time; clonazepam 0.5-1.0 mg/time; or phenobarbital 15-30 mg/time, 1/4-1/2 tablet each time, 3 times a day, and the maximum dose is no more than 1 tablet/time, 3 times a day. The main adverse reaction is drowsiness.

4. Other drugs: Calcium channel blockers: flunarizine, nimodipine, nifedipine, and carbonic anhydrase inhibitor methazolamide are effective for some patients.

II. Nursing of essential tremor

1. Physical exercise: The nursing principle for patients with essential tremor should be to try to do some physical exercise, such as Tai Chi and radio gymnastics. Rubbing your hands and rotating your neck can help promote blood circulation.

2. Diet: Eat more foods that resist arteriosclerosis, such as vegetables, fruits, celery, onions, etc., which have the effect of lowering blood pressure and anti-blood lipids. Eat more whole grains. The richer the variety, the better for recovery. High-fat, high-calorie, and high-sugar foods should be limited, especially fried foods should be eaten less. Maintain a regular daily life and avoid fatigue.

3. Psychological treatment: Patients should pay attention to mental balance, read more books, use their brains more, and go out often. Some patients have heavy mental burdens and are afraid of being laughed at by others. At this time, family members, relatives and friends should give him comfort and encouragement. Do not make the patient angry or nervous, especially if there is some bad news at home. Do not tell him immediately to avoid worsening of the condition. Create a warm and peaceful recovery environment.

4. Strengthen limb function training: In the early stage of this disease, you should insist on certain physical activities, actively perform limb function exercises, and perform flexion, extension, rotation and other activities in the joints of the limbs to the maximum range to prevent limb contracture and joint stiffness. Patients in the advanced stage should perform passive limb movements and massage their muscles and joints to promote blood circulation in the limbs.
