What examinations should be done for lumbar disc herniation?

1. Physical examination: Most patients with lumbar disc herniation can be correctly diagnosed based on clinical symptoms or signs. The main symptoms and signs are:

① Low back pain combined with sciatica radiating to the calf or foot, positive straight leg raising test;

② There is obvious tenderness on the side of the L4-5 or L5-S1 interspinous ligament, and there is also radiating pain to the calf or foot;

③ Decreased sensation of the skin on the anterior or posterior lateral side of the calf, decreased strength of the toe muscles, decreased or absent Achilles tendon reflex on the affected side. X-rays can rule out other bone lesions.

2. Imaging examination:

1. X-ray examination: The lumbar and sacral spine should be taken in the AP and lateral views, and left and right oblique views should be taken when necessary. Scoliosis is often seen, and sometimes the intervertebral space narrowing and lip-like hyperplasia of the vertebral edge can be seen. Although X-ray signs cannot be used as a basis for diagnosing lumbar disc herniation, they can be used to rule out some diseases such as lumbar tuberculosis, osteoarthritis, fractures, tumors and spondylolisthesis.

2. CT and MRI examinations: When it is difficult to diagnose severe patients or atypical cases, special examinations such as spinal iodized oil contrast CT scan and magnetic resonance imaging can be considered to clarify the diagnosis and the protruding part.

Patients with no obvious abnormalities in the above examinations cannot completely rule out lumbar disc herniation.
