What is the relevant introduction about blood amylase?

Blood amylase is the main type in serum. Blood amylase is mainly derived from the pancreas. A small amount of secretion also occurs in the proximal duodenum, lungs, uterus, and breast. It is a combination of various foods and helps the body play an important role in digestion. The main function of blood amylase is to treat acute pancreatitis. It has a good diagnostic effect. However, it should be noted that this drug looks complicated and it is difficult to fully understand it at once. In normal times, we should try to understand it from multiple aspects and know the principles of medication.

Normal serum amylase (AMS) values

(1) Enzyme rate method (37°C): serum: 20~90U/L;

(2) Iodine colorimetry: serum: 800~1800U/L;

(3) BMD method:

Adult: 25~125U/L;

Adults > 70 years: 28~119 U/L.

(Note: The specific reference value depends on each laboratory.)

Clinical significance of serum amylase (AMS)

(1) Hyperamylasemia:

① Increased pancreatic amylase:

A. Pancreatic diseases: acute pancreatitis, acute exacerbation of chronic pancreatitis, pancreatic cysts, pancreatic cancer, gastroduodenal diseases, etc.

B. Drugs: pancreatic hormone, incretin, cholecystokinin, drug-induced pancreatitis, thiazides, steroids, etc.

C. Pancreatic juice leaks and is absorbed from the digestive tract, causing digestive tract perforation, intestinal necrosis, peritonitis, penetrating ulcers, etc.

D. Pancreatic juice reflux: ERCP (endoscopic pancreaticobiliary duct examination) and other pancreatic and bile duct examinations.

② Increased salivary gland amylase: salivary gland diseases, tumors producing amylase, postoperatively, shock, burns, etc.

③Amylase levels in both increased: chronic renal insufficiency.

④ Macroamylasemia: The cause is unknown, after intravenous injection of hydroxyethyl starch.

(2) Hypoamylasemia:

① Decreased pancreatic amylase: pancreatic disuse caused by pancreatic diseases (total pancreatectomy, extensive pancreatectomy, acute fulminant pancreatitis), severe diabetes, severe liver disease, etc.

② Decrease in salivary gland amylase: after salivary gland removal and irradiation.

Serum amylase (AMS) precautions

1. The use of oral contraceptives, codeine, morphine, anesthetics, sulfonamides and saliva-contaminated specimens will cause the specimen test results to be low.

2. The blood test results have little to do with eating, and there is no need to be on an empty stomach for blood drawing.
