Differential Diagnosis

1. Infectivity

Infectious fever often has the following characteristics:

(1) Acute onset of fever with or without chills

(2) Systemic and localized symptoms and signs

(3) The white blood cell count in the blood is higher than 12×10/L, or lower than 5×10/L.

(4) Nitrogen blue tetrazolium test (NBT): If the neutrophil reduction of NBT exceeds 20%, it indicates bacterial infection, which helps to differentiate from viral infection and non-infectious fever. It may be false negative after the use of hormones.

(5) A positive C-reactive protein (CRP) test indicates bacterial infection and rheumatic fever, while a negative test usually indicates viral infection.
