Purulent parotitis is also a relatively common type of mumps. Patients will have serious infections, which will lead to some retention and congestion. There are many causes, such as poor oral hygiene. Bacteria enter the gland along the parotid duct and cause inflammation. At this time, timely treatment is needed. First, we must understand the pathogenic bacteria, and then carry out symptomatic anti-inflammatory treatment. In normal times, we should pay attention to oral hygiene, brush our teeth in time, and gargle well. Treatment of suppurative mumps Acute suppurative parotitis should be treated with broad-spectrum, anti-Gram-positive coccal antibiotics as early as possible. Local physical therapy can be used, such as ultrashort wave, infrared, or external application of Chinese medicine. Rinse locally to clean the mouth. If the inflammation cannot be controlled after conservative treatment, the patient will experience throbbing pain, local pitting edema, or compression of the parotid tissue, with pus flowing out of the parotid duct. On the one hand, the pus should be cultured for drug sensitivity, and on the other hand, incision and drainage should be performed. At the same time, pay attention to systemic supportive treatment, including water, electrolyte, and acid-base balance. Chronic suppurative parotitis can be treated conservatively. If there is clear presence of stones, the parotid main duct is enlarged and pus is accumulated, and swelling occurs frequently, parotidectomy to preserve the facial nerve may be considered. Prevention of suppurative mumps Maintaining oral hygiene is an important part of preventing the onset of suppurative mumps. Brush your teeth every morning and evening, rinse your mouth after meals, and perform periodontal cleaning when necessary. Patients with weak constitution, long-term bed rest, high fever or fasting may often suffer from dehydration. They should strengthen oral care (such as brushing teeth carefully, gargling with chlorhexidine solution frequently, etc.), maintain fluid balance, and strengthen nutrition and anti-infection treatment. After major surgery, especially abdominal surgery, the reflex function of the salivary glands may be reduced or stopped, and care should be taken to prevent the occurrence of acute suppurative parotitis. Clinical manifestations Acute suppurative parotitis often occurs on one side. The onset is sudden, and most patients have systemic symptoms such as high fever, chills, general malaise, and leukocytosis. In a few patients, the above systemic reactions may not be obvious. The parotid area on the affected side was obviously red and swollen, the retromandibular depression disappeared, and the earlobe turned up. Because the parotid gland capsule is dense, the swelling is restrained, the internal pressure increases, the pain is severe, and the tenderness is obvious. There is varying degrees of restricted mouth opening. The opening of the parotid duct on the affected side is red and swollen, and purulent secretions are discharged. Due to fascial separation, abscesses are often multiple, scattered small pus foci, so there is no typical sense of fluctuation in the early stage. Parotid gland pus can penetrate the parotid gland fascia and enter adjacent tissues or spaces, such as the external auditory canal and parapharyngeal space. Chronic suppurative parotitis is often bilateral, and most patients seek medical attention due to recurrent parotid swelling. Episodes of swelling are sometimes associated with eating and are mildly painful. Generally there are no systemic symptoms. The parotid gland is slightly swollen or not obvious, the duct opening may be slightly red, and turbid saliva may flow out when the gland is squeezed. In patients with a long course of illness, the gland feels hard and tough, and the parotid duct appears to be thick and hard cord-like. |
