Why do tooth cavities occur?

Tooth cavities are mainly caused by caries. There are four conditions for the occurrence of dental caries: bacteria, food, teeth, and time.

Under normal circumstances, there are many bacteria in people's mouths, one of which is acid-producing bacteria called Streptococcus mutans, also known as cariogenic bacteria. When food residues remain on the surface of teeth for a long time, caries-causing bacteria will react with carbohydrates in the food to produce acids. These acids react with the enamel, causing the enamel surface to decalcify, soften, and disintegrate, forming shallow cavities. At this time, there is generally no feeling of pain and it is not easy to be detected. But if left untreated, the cavity will become larger and deeper, damaging the shallow layer of dentin. At this time, your teeth will feel sore when you eat hot, cold, sour, or sweet stimulating foods. If it is still left untreated, the pulp will be damaged, causing pulpitis, and spontaneous severe pain will occur.

1. Shallow caries

Generally, there are no obvious caries holes, only a rough feeling when probing. In the later stage, shallow holes limited to the enamel may appear, with no subjective symptoms and no response to probing.

2. Medium caries

Caries has reached the superficial layer of dentin. Clinical examination shows obvious cavities. There may be tenderness on probing. There may be pain response to external stimuli (such as cold, heat, sweet, sour and food embedded in the teeth). The pain disappears immediately when the stimulus is removed. There is no spontaneous pain.

3. Deep caries

Caries has reached the deep layer of dentin, generally manifesting as large and deep cavities, or small entrances with more extensive damage in the deep layers. The reaction to external stimuli is more severe than that of moderate caries, but after the source of stimulation is removed, the pain can still be relieved immediately, and there is no spontaneous pain.

Caries initially manifests as a small black spot on the tooth, which is often ignored because there are no obvious symptoms to warn us. In many cases, we just ignore it. However, if it is not treated in time, it will slowly develop into a black hole. At this time, the symptoms will be very obvious, and sometimes the pain is unbearable. If it continues to develop, it will lead to pulpitis and apical periodontitis, and even cause inflammation of the alveolar bone and jaw. Secondary infection of dental caries can form lesions, leading to other systemic diseases such as arthritis, periostitis, chronic kidney disease and various eye diseases.

Therefore, if you have a cavity in your tooth, you cannot ignore it. You must go to the dental department for examination and treatment in time. Even if it is only superficial caries, we must actively treat it to nip the caries in the bud.
