What will happen to palmoplantar keratosis?

Palmoplantar keratosis is a genetic disease. Patients mainly experience genetic thickening of the skin on the palms or feet, with severe peeling. The skin texture appears particularly deep, and the palms and soles are very rough and over-aged. Moreover, the thickened stratum corneum will grow back not long after being removed, causing great pain to patients. So, how should palmoplantar keratosis be treated, and what will be the final outcome?

Palmoplantar keratosis is a hereditary skin disease characterized by excessive keratinization of the palms and soles of the feet, so it is also called hereditary palmoplantar keratosis. It is also a general term for palmoplantar keratosis diseases. The disease is autosomal dominant and has a clear family history. Palmoplantar keratoderma is an autosomal dominant genetic disease with obvious familial characteristics. It can be passed down from generation to generation or terminated after several generations, and there is no positive family history to be traced. The disease can occur at an early age, or in childhood or adolescence, but there is a family history. The disease can also be symptomatic, occurring in keratosis follicularis and pityriasis rubra pilaris. This disease cannot be cured because it is hereditary.

There is no effective treatment for this disease. Because the skin will proliferate and become keratinized during wound repair, trauma should be avoided. Topically apply 0.1% vitamin A acid ointment, 5%~10% salicylic acid ointment or 15%~20% urea ointment to relieve symptoms. You can also try taking vitamin A. Radiation therapy works temporarily but not permanently.

Oral isotretinoin or acetylcholine ester tablets can reduce keratinization and hypertrophy during medication, but will recur soon after stopping the medication. Taking vitamin A and vitamin E is also not effective. Generally, the focus is on local topical medication treatment. 0.1% retinoic acid ointment, 10% urea ointment, 10% salicylic acid ointment, or 0.25% anthralin ointment can be used, but packing treatment is required, especially at night, which will achieve the effect of exfoliation. You can also try glucocorticoid ointment pack treatment. It is also recommended that 6% salicylic acid propylene glycol gel can be used topically to achieve good therapeutic effects.

Patients with hereditary diseases should try to marry someone from another place to reduce the chance of inheriting the disease. Retinoic acid derivatives should be taken for treatment. After applying topical retinoic acid ointment, the condition was alleviated and improved, but it relapsed immediately after stopping the medication. Gene therapy may be used in the future.

Patients with palmoplantar keratosis usually need to apply ointment to the palms and soles of the feet to prevent chapped skin.
