Causes (1) The suture has not fallen off The sutures around the stoma have not completely fallen off, and the suture stimulation causes inflammatory changes in the stoma mucosa, tissue cell proliferation, and granulomas in the mucosa around the stoma. (2) Improper chassis cutting Improper cutting of the stoma base plate, or too small or uneven edges, stimulates the proliferation of fibrous tissue and produces granulomas in the mucosa around the stoma. Clinical symptoms Granulomas are benign tissues. It is red granular. Bleeding is easy to touch, usually occurring at the mucosa-skin interface and growing around the edges of the stoma. Related care 01 Nursing Assessment Methods Assess the patient's stoma care method, whether the size of the inner ring of the stoma base is appropriate to the stoma diameter; whether the clothes are too tight; whether the stoma mucosa is cleaned too frequently; whether the items used to clean the stoma mucosa are rough and not soft enough. The above factors are the reason why the stoma mucosa is often stimulated by the friction of the stoma base plate and bag body, inducing granuloma. 02 Pain care When in pain, patients can distract their attention, such as listening to music or talking about things that interest them, and receive psychological counseling to relieve tension and alleviate pain. 03 Nursing measures 1. If granulation occurs at the contact point between the mucosa and the skin, check whether there are sutures around the stoma that have not fallen off. If knots are observed, they should be removed, and then use stoma skin care powder, apply stoma anti-leakage ointment, and stick the stoma bag. Change the stoma bag every two days to protect the wound from contamination. 2.
