Is the skin around your stoma itchy?

Acne and itching around the stoma are likely caused by fungal infection. If you want to relieve the symptoms, you must actively treat anti-fungal infections and keep the local area clean and dry. You must strengthen your nutrition and maintain a happy mood. You also need to develop good living habits and eating habits, eat less spicy and irritating foods, try to eat light foods, and be nutritionally balanced.

Peristomal skin disease can be treated as follows:

(1) Ostomy devices need to be replaced if they are caused by the device.

(2) Contact dermatitis should be treated with skin protective ointments, oils and other drugs (such as zinc oxide ointment, etc.).

(3) For patients with stubborn erosions and ulcers caused by fungal infection, apply topical nystatin.

(4) If various treatments are ineffective, surgery to change the stoma position may be considered.

Stoma mucosal granulomas are benign tissues that usually occur at the contact point between the mucosa and the skin, but can also occur on the stoma mucosa, with one or more granules growing around the edge of the stoma. Stoma mucosal granuloma not only causes pain and itching to patients, which is easily accompanied by bleeding, but also affects the adhesion of the ostomy bag, easily causing leakage and leading to the occurrence of peristomal dermatitis. Foreign bodies and external mechanical stimulation cause mucosal damage, inflammatory changes, tissue cell proliferation, and granuloma formation.

Causes

(1) The suture has not fallen off

The sutures around the stoma have not completely fallen off, and the suture stimulation causes inflammatory changes in the stoma mucosa, tissue cell proliferation, and granulomas in the mucosa around the stoma.

(2) Improper chassis cutting

Improper cutting of the stoma base plate, or too small or uneven edges, stimulates the proliferation of fibrous tissue and produces granulomas in the mucosa around the stoma.

Clinical symptoms

Granulomas are benign tissues. It is red granular. Bleeding is easy to touch, usually occurring at the mucosa-skin interface and growing around the edges of the stoma.

Assess the patient's stoma care method, whether the size of the inner ring of the stoma base is appropriate to the stoma diameter; whether the clothes are too tight; whether the stoma mucosa is cleaned too frequently; whether the items used to clean the stoma mucosa are rough and not soft enough. The above factors are the reason why the stoma mucosa is often stimulated by the friction of the stoma base plate and bag body, inducing granuloma.

02 Pain care

When in pain, patients can distract their attention, such as listening to music or talking about things that interest them, and receive psychological counseling to relieve tension and alleviate pain.

03 Nursing measures

1. If granulation occurs at the contact point between the mucosa and the skin, check whether there are sutures around the stoma that have not fallen off. If knots are observed, they should be removed, and then use stoma skin care powder, apply stoma anti-leakage ointment, and stick the stoma bag. Change the stoma bag every two days to protect the wound from contamination.

2. Measure the stoma size correctly to avoid the base plate frequently rubbing against the stoma edge, which may cause granulation tissue hyperplasia.
