What medicine to take for HP infection

1. Indications for H. pylori eradication

The following diseases that are Hp positive are all treated to eradicate Hp.

(1) Peptic ulcer, whether initial or recurrent, active or dormant, with or without bleeding.

(2) Gastric MALT lymphoma.

(3) Gastritis with severe abnormalities such as erosion, intestinal metaplasia, and atypical hyperplasia.

(4) After resection of early gastric cancer.

2. Treatment options for eradicating H. pylori

Currently, there are two major treatment options for eradicating H. pylori, namely, triple therapy based on proton pump inhibitors (PPIs) and colloidal bismuth (CBS) plus two antimicrobial drugs. The standard dose of PPI is omeprazole (Losec) 20 mg or lansoprazole (Dakprom) 30 mg; the standard dose of bismuth is potassium citrate bismuth (Weiyangling) 240 mg (2 capsules). Commonly used antibacterial drugs include amoxicillin, clarithromycin, metronidazole or tinidazole, furazolidone and tetracycline. National HP experts recommend the following treatment options for clinical selection

(1) PPI-based triple therapy for seven days: ①OMC250: omeprazole 20 mg + metronidazole 400 mg + clarithromycin 250 mg, all taken twice a day, for 7 days. ②OAC500: omeprazole 20 mg + amoxicillin 1000 mg + clarithromycin 500 mg, all taken twice a day, for 7 days. ③OAM: omeprazole 20 mg + amoxicillin 1000 mg + metronidazole 400 mg, all taken twice a day, for 7 days.

(2) CBS-based triple therapy for 14 or 7 days: ① BMA: colloidal bismuth 240 mg + metronidazole 400 mg + amoxicillin 500 mg, all taken twice a day, for a course of 14 days. ②BMT: colloidal bismuth 240 mg + metronidazole 400 mg + tetracycline 500 mg, all taken twice a day, for 14 days. ③BFC250: bismuth 240mg + furazolidone 100mg + clarithromycin 250mg, all taken twice a day, for 14 days. ④BMC250: bismuth 240 mg + metronidazole 400 mg + clarithromycin 250 mg, all taken twice a day, for 7 days. The above-mentioned triple therapy is currently the first-line treatment option commonly used in clinical practice, and the Hp eradication rate can reach 80% to 90%. For economic reasons, the PPI in PPI triple therapy can be replaced with H2 receptor antagonist (H2RA), but the efficacy is slightly worse.
