The scrotal skin of primary hydrocele is normal, has high tension and is translucent. If the tension of the tunica vaginalis is not great and it is relatively soft, one should consider the possibility of secondary hydrocele and be alert to the presence of lesions in the testicles and epididymis, such as tuberculosis, syphilis, inflammatory lesions, tumors and filariasis. Causes If an adult has a small amount of hydrocele and no discomfort symptoms, it is often discovered accidentally during a physical examination; those with a large amount of hydrocele often feel scrotal sagging, swelling, spermatic cord traction pain, etc. When there is hydrocele of giant testicles, the penis retracts into the foreskin, affecting urination and sexual life, and also making walking and labor inconvenient. In adults, hydrocele can form the following types of hydrocele due to incomplete closure of the processus vaginalis at different locations: 1. Testicular hydrocele: The processus vaginalis is closed normally, but there is a lot of fluid in the testicular hydrocele sac. Because the testicles and epididymis are wrapped, the testicles cannot be touched during physical examination. It can be divided into primary and secondary. The former is of unknown cause, while the latter is caused by inflammation, trauma, tumors and filariasis. It is the most common type of hydrocele; 2. Spermatic cord hydrocele: The two ends of the processus vaginalis are closed, but the spermatic cord sac in the middle is not closed and has fluid accumulation. The fluid accumulation is not connected to the abdominal cavity and the spermatic cord sac, which is also called spermatic cord cyst. It grows along the spermatic cord, and normal testicles and epididymis can be felt below it. If the testicle on the same side is pulled, the cyst can be seen moving up and down. 3. Hydrocele of the testicle and spermatic cord (infantile type): The processus vaginalis is closed at the inner ring, but not at the spermatic cord, and is connected to the testicular sac. The appearance is pear-shaped. Although the outer ring opening is enlarged due to the pressure of the effusion, it is not connected to the abdominal cavity. 4. Communicating hydrocele (congenital): The processus vaginalis is not completely closed, and the fluid in the hydrocele sac can communicate with the abdominal cavity through a small tube, which is also called congenital hydrocele. Sometimes the intestine or greater omentum may enter the sac, which is called congenital inguinal hernia. |
