1. Conservative treatment

In the early stage of femoral head necrosis, pain is the main symptom. The treatment plan for traumatic femoral head necrosis is as follows:

(2) Perform femoral head drilling decompression surgery. If the adductor muscle is spasmodic, adductor muscle release surgery can be performed.

(3) For those with limited hip joint mobility, start self-functional exercises for the hip joint.

2. Mid-stage femoral head necrosis

(1) Can be combined with physical therapy and hyperbaric oxygen therapy.

(2) Depending on the severity of the lesion, femoral head bone grafting and adductor muscle release can be used.

3. Surgery

Core decompression can be performed for minor lesions; however, vascularized bone grafting can be performed for larger lesions or mild collapse; and total hip replacement is the only option for advanced cases.

At present, the most ideal way to treat mid-to-late stage femoral head necrosis is artificial femoral head replacement. For patients in their 40s or 50s, this approach is often used if the surgical indications are appropriate. If the patient's condition continues to progress after the above treatment, and the patient is older, artificial femoral head replacement can be considered.

In Western medicine, most patients undergo surgical treatment for femoral head necrosis, except for a small number of patients who use non-surgical and interventional therapies. Although various surgeries have their advantages and disadvantages, they are not easily accepted by patients due to their high costs and wide limitations. Moreover, patients with other surgical contraindications and the elderly and young patients are not suitable for surgical treatment.
