1. Scrotal symptoms Scrotal itching is the initial symptom, which is particularly severe at night and can affect sleep in severe cases. Scrotal lesions can be roughly divided into 3 types.

(1) Erythematous type: Symmetrical distribution of erythematous patches on both sides of the scrotum. The patches vary in size and are more than 2 to 3 cm in diameter. The patches are shiny and have adherent grayish white scales and crusts. They are wrinkle-free and have no infiltration. They are slightly higher than the skin surface and are clearly demarcated from the surrounding skin. In patients with a longer course of illness, the erythema appears dark red. The same lesions can be seen at the end of the foreskin, that is, there is a thick, brown-black, sticky scab on the foreskin at the glans penis, with obvious and neat edges. Erythematous changes account for about 2/3 of patients with scrotal dermatitis.

(2) Eczema type: Its symptoms are indistinguishable from those of general eczema. The skin lesions are characterized by dryness, desquamation, crusting, infiltration, hypertrophy, and deep wrinkles. Severe cases include exudation, erosion, fissures or suppuration. When touched by hand, it feels like rubber, and its edges are diffuse or localized. The range of skin lesions may only cover 1/3 of the scrotum, while others may involve the scrotum and perineum.

(3) Papular type: The skin lesions are characterized by scattered or densely clustered red flat papules ranging in size from mung beans to soybeans, asymmetrically distributed on both sides of the scrotum and covered with shiny scales. A few cases present with lichenoid lesions.
